1. Lake Stockton Healthcare Facility, Stockton, MO:  A complaint investigation/survey was conducted.  The facility failed to ensure one cognitively impaired resident in the special care unit was free from sexual abuse from a resident who displayed sexual and physical aggressive behaviors towards other residents.
  2. Golden Years Center for Rehab and Healthcare, Harrisonville, MO: A complaint investigation was completed. The facility failed to ensure the communicator of the facility’s fire alarm system sent a signal to a Continuously Attended Alarm Monitoring Station, due to the disconnection of phone service for the dedicated alarm phone lines due to nonpayment.
  3. The Gables at Brady Circle, LLC, St. Louis, MO: A complaint investigation was conducted.  The facility failed to provide protective oversight for one cognitively impaired resident who eloped wearing only a brief and t-shirt.  Staff also failed to provide protective oversight for four residents who were unattended when staff responded to the resident’s elopement.
  4. Edgewood Manor Center for Rehab and Healthcare, Raytown, MO: A complaint investigation was completed. The facility failed to ensure the communicator of the facility’s fire alarm system sent a signal to a Continuously Attended Alarm Monitoring Station, due to the disconnection of phone service for the dedicated alarm phone lines due to nonpayment.
  5. Big Spring Care Center for Rehab and Healthcare, Humansville, MO: A complaint investigation was completed.  The facility failed to ensure the communicator of the facility’s fire alarm system sent a signal to a Continuously Attended Alarm Monitoring Station, due to the disconnection of phone service due to non-payment.  The facility failed to complete an approved fire watch while the phone line utilized by the fire alarm system failed to work properly due to nonpayment of the phone bill.  The facility also failed to notify the local fire authority and DSS when he fire alarm system did not function properly.
